In this paper, I create a model health assessment that is holistic in content and application and introduce the nature and benefits of a healing relationship between health practitioner and patient. This holistic assessment is discussed within the dominant, dualistic worldview and the emerging, holistic worldview. Existing health assessments are noted for their limited assessment of what influences disease and healing. The assessment aims to expand the boundaries of how illness and health are shaped, perceived, and experienced. It assesses the quality of the whole person's (personal, social & environmental) life, instead of being limited to identifying disease or illness. This model addresses more than disease management and prevention; it is a tool for personal development and healing.
